Ojala, P.E., P.L.S. Arne H. Ojala, P.E., P.L.S. Daniel E. Gonsalves, P.E., S.E. Craig J. Ferrari, E.I.T., S.E. Phellyp T. Tonia!, E.I.T., S.E. In December 2022, the Commission issued an Order of Conditions for the above-referenced site, for the construction of an addition to an existing single-family dwelling. A site visit was made and a survey of the existing conditions relative to the Order was performed. All work to the house has been completed and roof drywells have been installed to infiltrate runoff from the new addition. The lower portion of steps nearest the wetland were retained for access from the deck to the lower lawn area. The brick patio was replaced with new pavers in approximately the same footprint. All other portions of this project conform to the approved site plan. An as-built site plan and photos are enclosed for review. This is to certify, to the best of my knowledge and belief, that the work was performed in substantial compliance with the approved plan and Order of Conditions aside from the above mentioned deviation.
