As requested by Wright-Pierce, S.W. COLE rep Andrew Carter, Construction Services
Technician, arrived onsite at approximately 6:55 AM to observe concrete placement operations
and perform field testing.
Upon arrival, the S.W. COLE representative met with Allen Sulc, representative of Wright-
Pierce, to discuss the planned concrete placement activities for the day.
Concrete was supplied by Cape Cod Ready Mix. The concrete was reported to consist of a 3/4-
inch nominal aggregate, 4,500-psi design mix. Approximately 99 cubic yards of concrete were
observed to be placed during the observation period.
Concrete sampling and field testing were performed in general accordance with applicable
ASTM standards. Samples were obtained from the first load delivered to the site. The following
results were obtained:
First Poured Load – Truck No. 74
Slump: 5.25 inches
Air Content: 3.3%
Concrete Temperature: 81°F
Concrete placement was observed to be performed using a pump truck. Internal consolidation
was observed using a worm-type internal vibrator. Concrete spreaders were observed to
distribute the concrete, and hand trowels were observed to level and finish the surface.
At the time of departure, the two columns located east of the headworks building and the
process footing support wall had not yet been placed. The S.W. COLE representative departed
the site prior to completion of all planned concrete placement activities in order to report to
another scheduled project. Prior to departure, Allen Sulc verbally indicated that no additional
concrete testing was requested for the remaining concrete placement activities. Although only

approximately 99 cubic yards of concrete placement were observed by the S.W. COLE
representative, Allen Sulc indicated that the planned placement quantity would exceed 100
cubic yards.
A total of five (5) concrete cylinders were cast from the first load for laboratory compressive
strength testing, consisting of one (1) cylinder designated for testing at 7 days, one (1)
cylinder designated for testing at 14 days, and two (2) cylinders designated for testing at 28
days. One (1) additional cylinder was designated for hold purposes.
Field test results and observations were verbally communicated onsite to Allen Sulc,
representative of Wright-Pierce, at the time of testing.
