To Whom it may concem:
The Yarmouth Fire Department responded to the Dollar General store located at 447 Route 28 in
west Yarmouth Massachusetts on July 23, 2026, at l238hrs for a smoke detector activation in the
rooftop unit #2. The store employees were notified to have the system serviced and the alarm was
silenced and left locked in. The Fire Dept. has since responded on July 24, 25,26,27 each day
around the same time just to silence the alarm that had resounded.
The Yarmouth Fire Department did respond to silence as a courtesy for the last 4 days to help
bridge the time needed to get a technician on site. That oractice or courtesv will cease as of
todav.
Violations to date:
527 CMR l.t3.l.lO No person shall shut off. disconnect. obstruct. remove. and/or modifu a fire
Drotection system or carbon monoxide Drotection system without first Drocuring a written permit in
accordance with Section l.l2 from the AHJ. The AHJ shall be notified when anv fire protection
svstem is out of service and on restoration of service.
No permit was issued for the system to be put on test at the central station, which is a modification
to the protection level.
527 CMR 1.13.1.8 All fire protection systems and devices shall be maintained in a reliable
operating condition and shall be replaced or repaired where defective or recalled.
The fire alarm system is still not repaired for the faulty smoke detector in RTU #2
527 CMR l.13.l.ll In the event of a failure ofa fire protection system or an excessive number of
accidental activations. the AHJ shall be permitted to require an aoproved fire watch until the svstem
is repaired.
You have 24 hours to have a plan in place for repairs to be made within 72 hours. Ifrepairs are not
made before Iuly 31, 2026, l200hrs. A fire watch will be ordered bv the fire department.
You are ordered;
I . to obtain a permit from the fire department to have the system taken offline.
2. Make repairs to fire alarm system within 72 hours
You are required to notify this office when corrections are made.
