Notes:
As requested by Rayssa Fonte, S.W.COLE rep was onsite to perform concrete field testing
associated with the concrete placement referenced above.
S.W.COLE Construction Services Technician Edwardo Baldera arrived onsite at approximately
5:00 a.m. to perform concrete field testing for the placement location referenced above.
Upon arrival, SWC rep met with Rayssa Fonte of CDM Smith, who provided an overview of the
planned concrete placement activities for the day.
The concrete for the wall placement was supplied by Cape Cod Ready Mix and consisted of a
4,500 psi concrete mix with 1½-inch nominal maximum coarse aggregate and Sika Control 75
admixture.
The concrete used for the column placement consisted of the same 4,500 psi mix design with
¾-inch nominal maximum coarse aggregate and the same specified slump and air-content
requirements.
Approximately 45 cubic yards of concrete were placed during the course of this operation,
consisting of approximately 41 cubic yards for the walls and 4 cubic yards for the columns.
Specified Concrete Properties
Slump: 7 to 10 inches
Air Content: 3.0% to 6.0% (air-entrained mix)
Concrete samples were obtained from the first load placed for the wall placement and from
the first load placed for the column placement. Field testing was performed in general

One set of five compression test cylinders was fabricated from each tested load for laboratory
compressive strength testing. Cylinders are scheduled for testing at 7 days, 14 days, and two
cylinders at 28 days. One cylinder from each set was retained as a hold specimen.
Test results and field observations were verbally communicated onsite to Rayssa Fonte of CDM
Smith during the course of our services.
