Notes:
As requested by CDM Smith, S.W.COLE rep Riley Nordahl was onsite to perform concrete
observation, field testing, and sampling at the referenced location.
The S.W.COLE rep arrived onsite at approximately 10:50 AM and met with Rayssa Fonte, a
representative of CDM Smith.
Concrete for this placement was supplied by Cape Cod Ready Mix. The approved mix design
indicated a design compressive strength of 4,500 psi and a nominal maximum aggregate size
of 1-1/2 inches. Approximately 10 cubic yards of concrete was reportedly ordered for this
placement.
Concrete placement activities were observed, and field testing and sampling were performed
as requested.
The approved mix design indicated a target slump range of 7 to 10 inches and a target air
content range of 3 to 5 percent.
Concrete field tests and samples were obtained from the first delivery truck during discharge.
Observed field test results were as follows:
Slump: 7.75 inches
Air Content: 3.5 percent
Concrete Temperature: 78°F
One set of five (5) concrete compressive strength test cylinders was cast from the first load for
laboratory testing. The cylinders were designated for one (1) 7-day break, one (1) 14-day

break, two (2) 28-day breaks, and one (1) cylinder to be held in reserve.
The cylinders were molded and cured and will be transported to the laboratory in general
accordance with applicable ASTM standards.
Prior to departing the site at approximately 1:15 PM, the S.W.COLE rep verbally communicated
the observed field test results and general observations to Rayssa Fonte of CDM Smith.
