BORDERING VEGETATED WETLAND DETERMINATION FORM
Project/Site: 24 Minnetuxet Way City/Town: Yarmouthport Sampling Date: 4/7/26
Applicant/Owner: Richard Calhoun Sampling Point or Zone: BVW
Investigator(s): Hamlyn Consulting Latitude / Longitude:
Soil Map Unit Name: Freetown 54A NWI or DEP Classification:
Are climatic/hydrologic conditions on the site typical for this time of year? Yes=No Q(If no, explain in Remarks)
Are Vegetation _7, Soil 0 or Hydrology significantly disturbed? (If yes, explain in Remarks) No
Are Vegetation = , Soil or Hydrology 0 naturally problematic? (If yes, explain in Remarks) No
SUMMARY OF FINDINGS — Attach site map and photograph log showing sampling locations, transects, etc.
Wetland vegetation criterion met? Yes No 0 Is the Sampled Area Yes= No
Hydric Soils criterion met? Yes ONo FI within a Wetland?
Wetlands hydrology present? Yes ✓=No 0
Remarks, Photo Details, Flagging, etc.:
Densely vegetated wetland borders White's Brook, extending from the perennial stream to
approximately contour 8 feet. The boundary of the wetland was delineated in the field at the limit of the
plant community that supports 50 or greater wetland indicator species, and saturatedconditions.

Remarks (describe recorded data from stream gauge, monitoring well, aerial photos, previous inspections, if available):
BVW can be delineated by vegetation alone, as the community transitions distinctly to upland indicator
species above elevation 8 feet.
1 his torm is only for BVW delineations. Other wetland resource areas may be present and should be delineated according
to the applicable regulatory provisions.
