Following up on our conversation in town hall: due to the Mass Leads Act, your permit 083-
2285 now expires on 8/23/2026. You must submit an after-the-fact ‘request to amend an
order of conditions’ to seek permitting for the unpermitted work with an as-built survey of
the current site conditions and a narrative of the unpermitted work. Please include the
outline of all planted mitigation areas on the new plan in addition to current grades and
disturbed areas, and the original work limit line. The checklist can be found linked below.
