AZ 14ALN IMtL PLAN c_3 0 Fire:l Permit to install tanks issued by fire Dept. Permit to store liquids is a separate permit. Fuel hoses should not extend beyond PLB grooves, vehicle collision protection, no suppression needed, Fire extinguishers, Emergency shut down and fuel spill kit to remain as is. Installed as per 527 CMR 1 and NFPA CL. Health: Please add to Sheet Number EQ-100 notation and illustration of: (1) double-walled tank construction and (2) signage for gasoline and diesel storage on the corresponding tank compartments. A Board of Health (BOH) meeting is required prior to the Zoning Board of Appeals (ZBA) meeting. The Health Dept. strongly recommends a Massachusetts Licensed Stie Professional (LSP) be engaged to test soils for contamination during the removal of the present UST installation. Please review below the requirements of the Town of Yarmouth, Board of Health Regulation: Handling and Storage of Toxic or Hazardous Materials with sections referenced. Please note these requirements overlap with some aspects of Fire regulation. Regulatory requirements and the applicable regulation section(s) are listed in bold font. An explanation of what is required or how the requirement is likely met, follows each regulatory requirement. Please be aware these are the same topics discussed during the conference call between the Health Dept.,DPW and Weston Sampson on 7-22-2026. No materials may be discharged to the environment.SECTION 9A This may seem obvious, but it includes UST sump or spill bucket wastewater which must be properly disposed. Methods of removal, storage and disposal:(1)staff pumps out the wastewater on an as-needed basis and stores it in a properly labelled drum until a contractor removes it for proper disposal;or(2)a contractor pumps the sump or spill bucket wastewater directly to a truck- mounted tank for immediate removal from the site and proper disposal. Please list the disposal procedure for sump wastewater. Outdoor storage of materials is prohibited.SECTION 9B This does not include UST or AST designed for outdoor placement. However, this does include replenishment fluids such as, but not limited to, windshield wash and DEF containers. If the fueling station has a canopy, and these items are placed a sufficient distance beneath the canopy, they are considered protected from weather. Am I correct in understanding a canopy will not be installed? I understand from the conference call on 7-22-2026 that no such storage is planned. Materials must be protected from vandalism.SECTION 9B I understand from the conference call on 7-22-2026 that vandalism protection includes electronic access by employees for each activation of fuel dispensing and the facility has a perimeter fence with electronically activated rolling gates that can only be opened by employees during off-hours. Materials must be protected from accidental damage.SECTION 9B I understand from the conference call of 7-22-26 that protective bollards will be installed in accordance with the UL 2085 standard, and I notice on Sheet EQ100, protective bollards surrounding the fuel tank are illustrated. Materials must be handled, stored, and used in such a way to prevent discharges. SECTION 10A To achieve this goal, an Emergency Spill Response Plan, unique to the facility, should be developed and spill control materials should be readily available. The Health Dept. can assist in the development of an Emergency Spill Response Plan by providing a one-page template in Word form that can be customized with a few edits. The Health Dept.emailed the spill plan template to DPW and Weston Sampson on 7-22-26. A sufficient supply of spill control materials must be kept readily available near fuel dispensers for use in the event of a fuel release or release of any toxic or hazardous material. For example,a covered container holding spill control materials compatible with gasoline and diesel, will cover this requirement. If the fueling station is near a storm drain,a storm drain blocker is recommended to be included with the spill control materials. Materials must be stored in covered product-tight containers. SECTION 9B and SECTION 10A New AST or UST have competent caps or lids. All materials must be stored inside secondary containment of 150% volume of the total amount of materials stored. SECTION 10C I notice that a new a new double-walled 10,000-gallon aboveground storage tank is listed within the Site Plan Review Cover Letter & Narrative. Double-walled tank construction substitutes for a 150% volume bermed containment area. Material containers must be properly labeled. SECTION 10D The tank compartments should be labeled for the type of fuel stored. If a drum of sump wastewater is stored while it awaits disposal, it should be properly labeled. All materials must be stored in such a manner to facilitate visual inspection of each container. SECTION 10D Of course, AST are visible and UST are not. In either case a continuous electronic leak detection system is required to monitor the condition of the tanks. I notice within the Site Plan Review Cover Letter & Narrative there is mention of a continuous electronic leak monitoring system that notifies the owner when a leak is detected. Any waste material must be properly disposed of by a licensed hazardous waste hauler. SECTION 10E This includes used fuel filters that I imagine are removed by the service company during the same visit when the new filters are installed. Any wastewater that accumulates in the sumps must also be properly disposed. Please explain the disposal procedure for used fuel filters which are likely removed by the service contractor immediately upon replacement. Water: Read & Rec ived Applicant(s)
