NOTICE OF DEED RESTRICTION

Notice is hereby given of the applicability of the Town of Yarmouth Board of Health for a deed restriction, to 15 Avery Lane , shown in Town Assessors Book dated 2010 , Map 99 ,
parcel 79. As Deed is recorded in the Land Court at the Barnstable County Registry of Deeds on the Deed Certificate # 191838 and Document # 1143328.
As plan of land is recorded in the Land Court at the Barnstable County Registry of Deeds, on a subdivision plan titled Plan 31209-D, Sheet 1, Barnstable Survey Consultants, Inc., Lot
76 and dated October, 1970.
The engineered plan prepared by Ronald J. Cadillac, PLS, RS, PC , dated July 29, 2010 , approved by the Health Department on August 9, 2010 , requires a maximum, not to
exceed:
(1) the number of bedrooms not to exceed Two ( 2 )
per design restrictions, Title 5, Section 15.214, Nitrogen Loading Limitations,
Zone II Areas of Wellhead Contribution.
