NOTICE OF DEED RESTRICTION

Notice is hereby given of the applicability of the Town of Yarmouth Health Department for a deed restriction, to 15 Jacqueline Circle , shown in Town Assessors Book dated 2016 , Map
29, Parcel 35. As Deed is recorded at the Barnstable County Registry of Deeds, on the Deed Book 2615 , Page 128 .
As plan of land is recorded at the Barnstable County Registry of Deeds on a subdivision plan titled “Grist Mill Village on Mill Creek Subdivision Plan of Land in West Yarmouth, Mass.
for John J. and Jacqueline F. Burke” Lot 4 dated September 18, 1959, and recorded on the Plan Book 151 Page 77.
The engineered plan prepared by Ronald J. Cadillac, PLS, RS, P.C. , dated October 12, 2016 approved by the Health Department on October 28, 2016 , requires a maximum, not
to exceed:
(1) the number of bedrooms not to exceed Two ( 2 ) per design restrictions,
