RE: Two Bedroom Deed Restriction for 15 Niagara Lane, West Yarmouth, MA
Dear Mr. Formisano:
As a reminder, this office sent to you a Two Bedroom Deed Restriction for 15 Niagara Lane, West Yarmouth to be processed as part of the final approval for the septic system upgrade for
the same address. Based on a telephone conversation with you on February 27, 2003, you are in receipt of the required paperwork, however, you wish to revise the Deed Restriction to
clarify the wording of the restriction. You were going to forward a proposed word change to this department for review in order to revise the restriction. This department has not
yet received your proposed changes.
Based on our telephone conversation and your intent to finalize the required paperwork in a timely fashion, the septic installation permit was issued. However, prior to any septic inspections
by this department, the deed restriction must be finalized and recorded at the Barnstable County Registry of Deeds and a copy of the recorded restriction must be submitted to the Health
Department.
