Notice is hereby given of the applicability of the Town of Yarmouth Board of Health Department for a deed restriction, to 22 Michelle’s Path , shown in Town Assessors Book dated
2002 , Map 76 , parcel 6 . As Deed is recorded in the Land Court at the Barnstable County Registry of Deeds on the Deed Certificate 170362 .
As plan of land is recorded in the Land Court at the Barnstable County Registry of Deeds, on a subdivision plan titled 35054-B (Sheet 2) .
The engineered plan prepared by Bennett & O’Reilly, Inc. , dated Revised February 17, 2004 , approved by the Health Department on February 19, 2004 , requires a maximum,
not to exceed:
(1) the number of bedrooms not to exceed Two ( 2 );
per design restrictions, Title V, Section 15.214, Nitrogen Loading Limitations,
Zone II Areas of Wellhead Contribution
(2) the installation of a MicroFAST® Unit 0.5 per DEP Provisional Approval Letter (Transmittal #W019014), dated January 27, 2004
