NOTICE OF DEED RESTRICTION

Notice is hereby given of the applicability of the Town of Yarmouth Health Department for a deed restriction, to 24 Shady Rest Drive , shown in Town Assessors Book dated 2016 , Map
98, Parcel 13. As Deed is recorded at the Barnstable County Registry of Deeds, on the Deed Book 7666 , Page 37 .
As plan of land is recorded at the Barnstable County Registry of Deeds on a subdivision plan titled “Section B, Fleetwood Park in (South) Yarmouth, Massachusetts, for Lads Investors,
Inc., Gerald A. Mercer & Co., Inc., Civil Engineers” Lot 102 dated June, 1964, and recorded on the Plan Book 187 Page 31.
The engineered plan prepared by Stephen A. Haas, P.E. , dated July 25, 2016 approved by the Health Department on August 3, 2016 , requires a maximum, not to exceed:
(1) the number of bedrooms not to exceed Two ( 2 ) per design restrictions,
(a) Title 5, Section 15.214, Nitrogen Loading Limitations
