Please find enclosed an application for a Two Bedroom Deed Restriction for the above address. Please complete the missing information on the application and return it to this office
for processing. Your design sanitarian can help you with this information, if needed. Once the application is processed, this office will forward the Deed Restriction to you for the
next step with additional instructions.
As part of the Title V variance approvals for the septic system installation and proposed addition to your property, the installation of a Norweco Singulair Model 960 under General Use
Approval is required with the following conditions. The Norweco Singulair unit must be operated and maintained by a certified operator with the following testing requirements: quarterly
testing for pH, BOD5, TSS, and Total Nitrogen of the system effluent is required. After one year of monitoring, at the request of the owner, the Health Department may reduce the monitoring
requirements.
