Notes:
As requested by CDM Smith, S.W.COLE rep. Riley Nordahl was onsite to perform concrete field-testing services
associated with the referenced concrete placement.
S.W.COLE Engineering Construction Services Technician Riley Nordahl arrived onsite at approximately 6:30 AM.
Upon arrival, the S.W.COLE rep. met with Rayssa Fonte of CDM Smith, who provided an overview of the planned
work activities for the day.
Concrete was supplied by Cape Cod Ready Mix and consisted of 3/4-inch, 4,500 psi concrete with Sika Control 75
as an additive.
Approximately 27 cubic yards of concrete were placed during the observation period.
Concrete samples for field-testing and fabrication of compressive strength specimens were obtained from Load 1
of the placement.
Field-testing services were performed in general accordance with applicable ASTM test procedures.
Concrete test specimens were fabricated from the sampled concrete for subsequent laboratory compressive
strength testing at one three day interval, one 7-day interval, one 14-day interval, and one
28-day intervals.
One additional specimen was retained as a hold sample.
Field Test Results – Load 1
Slump: 7.0 inches
Air Content: 5.0%
Concrete Temperature: 73°F

Based on the field-testing performed on the sampled concrete, the measured slump, air content, and
temperature values were observed to be within the project-specified acceptance ranges at the time of testing.
Test results and observations were verbally reported to Rayssa Fonte of CDM Smith while onsite.
Upon completion of the requested services, the S.W.COLE rep. departed the site at approximately 8:05 AM
