• All filings must be made on Town of Yarmouth forms.
• All applications must be submitted two weeks prior to the next scheduled Conservation
Commission meeting. Refer to Meeting Schedule.
• Failure to follow the Administrative Checklist shall result in an Administrative Incomplete
Application and will not be scheduled for a Public Hearing.
• Contact the Conservation Office if you need assistance.

Signed statement from an Engineer or similar professional who prepared the
approved plan stating the date of a recent site inspection and certifying compliance
with the permit and outlining departures from the approved plan, if any exist.
If mitigation planting was required, include planting date and confirm the required
time has elapsed*.
As -Built plan meeting all Plan Renuirements. — *Refer to the issued Order of
Conditions: Special Conditions to determine if an as -built is required*.
PDF of the COC request including application, letter, and as -built plan (if needed)

Local filing fee. Refer to the Fee Schedule. Make check payable to "Town of Yarmouth"
and include property address in memo.

In March 2024, the Commission issued an Order of Conditions for the above -referenced site,
for the construction of a seasonal dock.
A site visit was made and a survey of the existing conditions relative to the Order was performed.
All work has been completed, and the site was found to be in substantial compliance with the
approved Site Plans with no observed deviations.
This is to certify, to the best of my knowledge and belief, that the work was performed in
substantial compliance with the approved plan and Order of Conditions. We respectfully
request the issuance of a Certificate of Compliance.

B. Submittal Requirements
Requests for Certificates.of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
