an approval of any plans stamped by a registered professional engineer, architect, landscape
architect, or land surveyor?

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
Office(see http://www.mass.pov/eea/agencies/massdep/about/contacts/find-the-massdep-regional-office-
foryour-city-or-town.html).

RE: Request Certificate of Compliance, 206 Blue Rock Road, South Yarmouth,
Massachusetts, DEP File SE#83-2204; BSC Job #50312.01
Dear Ms. DiRienzo and Members of the Conservation Commission:
BSC Group, Inc. (BSC) is pleased to submit this Request for a Certificate of Compliance (RCOC)
for DEP File SE#83-2204 for the project authorized in the Order of Conditions (OOC) issued by
the Town of Yarmouth, DEP File SE#83-2204, for the property at 206 Blue Rock Road, South
Yarmouth (the Site). The OOC was issued on 8/6/2019.
A recent inspection has confirmed those planting and walkway issues with the original
application have been resolved and as such BSC is requesting the COO be issued.
Please review this Request for a Certificate of Compliance documenting compliance with the
Order of Conditions with those exceptions discussed and resolved. Please inform me if you have
