Notes:
As requested by CDM Smith., S.W. COLE rep. Henry Among, Construction Services Technician, was onsite to observe concrete
placement activities and perform field testing. Upon arrival, Henry spoke with CDM representative regarding the concrete
placement activities planned for the day.
Concrete was delivered by Cape Cod ready mix. The concrete was reported to be a 3/4-inch and 1-1/2 inch respectively of
nominal aggregate, 4500-psi design strength and sika control admixtures. Approximately 11 cubic yards of3/4 aggregate size
of concrete and 24 cubic yards of 1-1/2 aggregate size were placed during the observation period. Concrete sampling and
field testing were performed in general accordance with applicable ASTM standards.
.Samples were obtained from loads one of 3/4 aggregate size concrete and placed during the observation period. another
sample was obtain from load one of the1-1/2 aggregate size concrete .The following field test results were recorded:
For 3/4 aggregate size concrete
Slump (ASTM C143)
Truck 73 (Load 1): 3 inches
Air Content (ASTM C231)
Truck 73 (Load 1): 4.5%
Concrete Temperature (ASTM C1064)
Truck 73 (Load 1): 65°F
For 1-1/2 aggregate size concrete
Slump (ASTM C143)
Truck 67 (Load 1): 6.5 inches
Air Content (ASTM C231)
Truck 67 (Load 1): 4.5%

Concrete Temperature (ASTM C1064)
Truck 67(Load 1): 70°F
Concrete placement activities were observed utilizing a concrete pump. Internal consolidation activities were observed
utilizing worm vibrators, and the concrete was observed being placed in lifts and distributed using hand tools .
A total of twelve (6) 4-inch by 8-inch concrete cylinders were cast, consisting of six (6) cylinders from load one of the 3/4
aggregate size concrete and a total of five (5) 6-inch by 12-inch concrete cylinders were casted, from load one of the 1-1/2
aggregate size concrete. The cylinders were designated for laboratory compressive strength testing as follows:
One (1) cylinder for 7-day testing
One (1) cylinder for 14-day testing
Three (3) cylinders for 28-day testing
One (1) hold specimen.
Upon completion of day's activities, Henry communicated the results onsite with CDM Smiths representative
