Enclosed please find the following information concerning the Application for Approval
to Construct a Sewage Disposal System to serve the existing patio club building and a
proposed addition to the building at the Blue Water Resort.
1 . Two (2) completed Applications for Disposal Works Construction Permits.
2. Six (6) copies of a sketch plan for a sewage disposal system to serve the
facility referenced above.
3. One (1 ) check for the required filing fee.
4. One (1 ) set of floor plans for the existing building and proposed addition.
The design and sketch plan was prepared based upon information provided by DeWitt
Davenport in regards to a meeting between Mr. Davenport and yourself.

Information pertaining to tie on-site sewage deposal systems at the Blue Water Resort complex.

designed and/or conditioned by Massachusetts Department of Public Health permits to conform in
general with the requirements of Title V: the Massachusona Stela Envlronmer*tal Cade for
subsurface disposal of sanitary sewage. Specific design information on the sixth system that
presently serves two units of the buildinrj situated In the northeast portion of the site could not be
found. Based upon the weaguration of the system shovel on a plan dated May 18,1989,from Dawn
Cape Engineering,Me theoretical leaching capacity meats the Tale V flow requirements of the two
moms served.
The following Is a won of the systems based on a compilation of record information.
1. The design plans for Systems t and 2 were prepared by Everett Hinckley and dated September
1979. Each system Consists of a 2260-gallon septic tank end a leaching field with 1620 square
feet. The leeching facilities were Installed to replace a facility constructed in 1970t. The
theoretical flow capacity of Ow two leeching facilities is 3240 gallons per day.
2. System 3 was reconstructed In 1978 based upon design plans prepared by Down Cape
Engineering datedAprit 19,1976. Based upon the plans and Maseactuasetle Depertnmont of
Pubic Health renditions,the system eansisls ore 3000-gallon septic tank and*32.4-foot by 67-
foot leaaehine!scatty. The theoretical flow capacity of the System 3 leaching tardily is 2170
gallons per day.
3. Sham 4 was reconstructed as a repair in 1989, Based upon design and as-built plans prepared
by Down Cape Engineering dated May 18 and lure 15,1989,respectively,the installation
utilized the existing 3000-gallon septic tank and a new teaching facility with a bottom area of
14201 square feet. The theoretical flow capacity of the System 4 leaching facility is 1420 gallons
per day.
