The purpose of this report is to: • Document how the non-structural and good housekeeping practices over the last 12 months adhered to the approved O&M plan • Document any work that was completed to properly operate and maintain the stormwater control measures. The completed project was designed and constructed in compliance with the Massachusetts Department of Environmental Protection (MassDEP) Stormwater Handbook and the Town of Yarmouth Stormwater Management Bylaw to maintain or improve stormwater runoff quality and quantity. The Town of Yarmouth issued a stormwater management permit (SH2023-003) for the ERP Facility project on May 22, 2023. The stormwater management system components were maintained as recommended in the Massachusetts Stormwater Handbook and in conformance with the following special conditions of the Yarmouth stormwater management permit. Special Condition 2: Bi-annual sweeping of the parking lot areas shall be carried out to ensure adequate TSS removal rates are achieved. Special Condition 3: The owner shall provide quarterly inspections of catch basins and retain records to identify the depth of sediment below the outlet pipe. Based on those records, the owner shall clean the catch basins upon 50% sediment depth or once every two years, whichever occurs first. Tighe&Bond Long Term O&M Plan 3-1 Section 3 Stormwater Management System The on-site stormwater management system is comprised of catch basins, hydrodynamic separator, and infiltration basins. In general, runoff from the proposed work areas will be collected via catch basins and hydrodynamic separator, then directed to three infiltration basins where stormwater runoff will infiltrate. See the included Figure A in Appendix A for the location of the various components of the Stormwater Management System and see Appendix B for the approved Long-Term Pollution & Operation and Maintenance plan. 3.1 Inspections Inspections were performed in accordance with the Massachusetts Department of Environmental Protection (MassDEP) Stormwater Handbook and the special condition from the issued stormwater management permit. Figure 1, provided in Appendix A, identifies the location of each BMP that was inspected and maintained as described in this Section. All inspections are logged using the Inspection Forms provided in Section 5 or logged in an equivalent Inspection Form. The following stormwater management system features were evaluated during each inspection: 3.1.1 Infiltration Basins Inspection Frequency: Monthly Special Inspection Event(s): Rainfall greater than 3 inches Surface infiltration basins were inspected monthly for standing water and biannually for settling, cracking, erosion, leakage, vegetative cover condition, and sediment accumulation. Additionally, infiltration basins were observed to identify depths of sediment and occurrence of debris that would impact functionality. The outlet control structure, if applicable, was observed for signs of clogging during storm events and erosion. Inspections of the basin occur monthly, especially the sand bottom, which should remain clear of any trash, sediment, or debris. Any trash, sediment, or debris encountered was removed. The side slopes were mowed a minimum of twice annually. Heavy equipment was never operated within infiltration basins. 3.1.2 Deep Sump Catch Basins Inspection Frequency: Monthly Special Inspection Event(s): Rainfall greater than or equal to the 1-year 24-hour storm event. Deep sump catch basins were inspected once per month. The visual inspection did ascertain that the catch basin is functioning properly (i.e., no blockages or obstructions to the outlet and/or hood) and measured the amount of solid materials that have accumulated in the sump. This was done with a calibrated dipstick, tape measure, or other measuring instrument so that the depth of deposition in the sump can be tracked. Section 3 Maintenance Contacts Tighe&Bond D0225-44 Long Term O&M Plan 3-2 Inspections were completed visually from the ground level. Deep sump catch basins were cleaned upon 50% sediment depth or once every two years, whichever occurred first. Cleanings were also conducted at the end of the foliage and snow-removal seasons. Clamshell buckets can be used to remove sediment. However, vacuum trucks will remove more trapped sediment, are more expedient, and are less likely to damage hoods on outlet pipes. Disposal of sediment removed from catch basins was completed in accordance with local, state, and federal requirements. 3.1.3 Hydrodynamic Separators Inspection Frequency: Quarterly Special Inspection Event(s): Rainfall greater than or equal to the 1-year 24-hour storm event. Hydrodynamic separators were inspected monthly and after significant rain events. They were observed for clogging, trash accumulation, and signs of damage or mechanical failure. Any debris was immediately removed, and any organics lodged in the inverts were rinsed out as necessary. 3.1.4 Driveway, Walkway, and Parking Lot Sweeping Inspection Frequency: Bi-annually Special Inspection Event(s): Spring Snow Melt, Autumn Leaf Fall All pavement surfaces were inspected biannually for deterioration or spalling. Additionally, the pavement surface was regularly monitored to make sure it drains properly after storms. Cleanings were conducted on a quarterly basis to prevent clogging. For best management practices, high -efficiency vacuum sweeping machines should be used to clean and maintain the surface. \\Tighebond.com\data\Data\Projects\E\E5092 Eversource (L&P 2025-2028)\0010 Yarmouth Stormwater Inspections\O&M Documents\E5092-0010- Long Term Pollution Prevention and Stormwater Management OM Plan.doc APPENDIX A STORMWATER BMP LOCATION MAP NSTORMWATER BMP LOCATION MAPYARMOUTH STORMWATER INSPECTIONSYARMOUTH, MASSACHUSETTSSCALE:DATE:FIGURE:JULY 202511" = 30'INFILTRATION BASIN #1HYDRODYNAMICSEPARATORCATCH BASINCATCH BASINSINFILTRATIONBASIN #3INFILTRATION BASIN #2CATCH BASINSHYDRODYNAMICSEPARATORHYDRODYNAMIC SEPARATORHYDRODYNAMIC SEPARATORCATCH BASINHYDRODYNAMICSEPARATOR APPENDIX B SHIVE HATTERY LONG TERM POLLUTION & OPERATION AND MAINTENANCE PLAN WILLOW STREET PROPERTY LINE, TYP.PARKING SETBACKLINE, TYP.PROPERTY LINE, TYP.200' BUFFER FORRIVER, TYP.(FIELD LOCATED)PROPERTY LINE, TYP.ST CB A-2 (48" CATCH BASINSEE DETAILS 6 & 7/C501)ST CB B-3 (48" CATCH BASIN)ST MH C-2 (HYDRODYNAMIC SEPERATORSEE 1/C503OPEN GRATE)ST FES C-1 (18 '' RCP FES)ST FES B-1 (18 '' RCP FES)ST MH B-2 (HYDRODYNAMIC SEPARATORSEE 1/C503OPEN GRATE)ST FES B-4 (12 '' RCP FES)OVERFLOW FROM BASIN #2ENTERS THIS FESST CB E-3 (3' X 3' RECTANGUALR INTAKEW/CONCAVE 2' X 2' INLET FRAME & GRATE)ST FES A-5 (12 '' RCP FES)OVERFLOW FROM BASIN #3ENTERS THIS FESST MH A-1 (HYDRODYNAMIC SEPARATORSEE 1/C503OPEN GRATE)ST CB A-4 (48" CATCH BASIN)ST FES A-1 (18 '' RCP FES)ST CB E-2 (3' X 3' RECTANGULAR INTAKEW/2' X 2' INLET FRAME & GRATE)ST FES E-1 (15 '' RCP FES)ST FES A-3 (12 '' RCP FES)ST MH D-1 (HYDRODYNAMIC SEPARATORSEE 2/C50324" X 24" FRAME AND GRATE)ST FES D-1 (15 '' RCP FES)OVERFLOW FROM BASIN #2ENTERS THIS FESST P-C-1 83 LF18" RCP CLASS IV @ 0.31%ST P-A-2 167 LF15" RCP CLASS IV @ 0.25%ST P-B-2 165 LF15" RCP CLASS IV @ 0.35%ST P-B-1 83 LF18" RCP CLASS IV @ 0.26%ST P-B-3 156 LF12" RCP CLASS IV @ 0.73%ST P-E-3 159 LF8 '' PVC @ 0.10%ST P-C-2 173 LF15" RCP CLASS IV @ 0.30%ST P-A-5 330 LF12" RCP CLASS IV @ 0.32%ST P-A-4 35 LF15" RCP CLASS IV @ 0.50%ST P-A-1 83 LF18" RCP CLASS IV @ 0.31%ST P-E-2 159 LF8 '' PVC @ 0.10%ST P-E-1-B 35 LF15" RCP CLASS IV @ 0.25%ST P-A-3 149 LF12" RCP CLASS IV @ 0.87%ST P-D-1 10 LF15 '' RCP @ 0.68%RIPRAP, TYP.ST FES G-1 (12 '' RCP FES)ST P-G-1 11 LF12 '' RCP @ 0.50%ST MH G-2 (HYDRODYNAMIC SEPARATORSEE 2/C503OPEN GRATE)ST MH F-2 (HYDRODYNAMIC SEPARATORSEE 2/C503OPEN GRATE)ST P-F-1 5 LF15 '' RCP @ 1.45%ST FES F-1 (15 '' RCP FES)ST CB C-3 (48" CATCH BASIN)INFILTRATIONBASIN #1INFILTRATIONBASIN #3INFILTRATIONBASIN #2BASIN #1OVERFLOWSPILLWAY33 33 293031323129 30 31 32 29 30 31 31 30 29 34333335343031323330333334 33 32 31313233 33 33333332323131 3 0 31 32 ST P-E-1-A 288 LF15" RCP CLASS IV @ 0.25%ST MH E-4 (HYDRODYNAMIC SEPARATORSEE 5/C50324" ROUND SOLID COVER)DRAWN BY APPROVED BY ISSUED FOR ISSUE DATE FIELD BOOK: PROJECT NUMBER Printed:EX. 1OPERATION & MAINTENANCE PLAN EAW IJH PERMIT 05/01/2023 3172201210 --- YARMOUTH ERP EVERSOURCE ENERGY, INC. 484 WILLOW STREET, YARMOUTH, MA 02673 P:\Projects\CH\3172201210\Deliverables\Drawings\1_Civil\Exhibits\Operation & Maintenance Exhibit.dwg5/1/2023 3:13:25 PM APRELIMINARY - NOT FOR CONSTRUCTION 12341234BCDEFABCDEF440 NORTH WELLS STREET, SUITE 320 CHICAGO, ILLINOIS 60654 312.324.5500 | SHIVE-HATTERY.COM APPENDIX C OWNER’S MONTHLY BMP INSPECTION LOGS APPENDIX D STORMWATER BMP INSPECTION & MAINTENANCE LOGS APPENDIX E BI-ANNUAL STREET SWEEPING LOGS To: P.A.
