RE: Subsurface Sewage Disposal System Inspection Report, 69 Captain Shiverick Road,
South Yarmouth
Deaz Mr.and Mrs. Smith:
This department is in receipt of a subsurface sewage disposal system inspection report
regarding the above referenced property conducted by Troy M. Willia.ms of Troy
Williams Septic Inspections on February 6, 2002 and received in this office an Mazch 1,
2002.
The report identifies the septic system has having failed the inspection for the following
reasons:
1. Scum buildup was found on bottom of leach pit cover and above inlet line.
2. Bottom of leaching pit was found located in adjusted high groundwater
elevation.
� " In addition, it was noted tha.t a garbage'disposal was located within the kitchen. In order
to maintain the use of the garbage disposal, the new septic system must be designed to
accommodate the flow from the gazbage disposal. If you do not wish to continue the use �
of the garbage disposal, it must be removed and a copy of the receipt for the removal
must be provided to this office.
You have sizty (60) days from the da.te of this letter to obtain engineered plans and
upgrade your septic system to state Title 5 requirements. We are aware that you ha.ve -
retained an enginee��sc�ieduled a perc test with this office. - -
