Dear Christine Smith, i Our records indicate that the operation and maintenance contract with Coastal Engineering, Co. Inc. for your iinnovative/alternative wastewater treatment system may have expired or was cancelled as of March 31st,2015.To date we I have not received evidence that you have entered into a new operation and maintenance contract. I I am writing to remind you that the Massachusetts Department of Environmental Protection (MA DEP) and the Town of Yarmouth require you to keep an operation and maintenance (O&M)contract in effect at all times for your system. Information about these requirements may be found at https:/lseptic.barnstablecountyhealth.org. My department oversees VA septic system management and compiiance efforts for the Board of Health in your town.We are � authorized by your Board of Health to contact you to inform you of the above requirement and to request your compliance. jAccordingly, please forward a copy of a signed contract via mail,fax or e-mail within fifteen (15)days of receipt of this letter. Smith April 28, 2015 ATTACHMENT1 OPERATION AND MAINTENANCE SCOPE OF SERVICES The following is a summary of the scope of services to be provided by Coastal Engineering Co., Inc. (CEC): i The treatment system shali be operated 6y a Certfied Wastewater Plant Operator in accordance with the requirements of 257 CMR 2.00 and fhe Board of Certification of Operators of Wastewater Treatment Plants.The freatment system shall I also be operated in accordance with the conditions established by the Massachusetts Departrnent of ErrvironmeMal i Protection(DEP) under 310 CMR 15.000 Title 5 of the Massachusetts Environmental Code for the pertnitted use and with the locai Board of Health. � EQUIPMENT MAINTENANCE 1. Within design capacity and capability of the equipment, maintain the system for the ben�t of Client. 2. Certify and d�ument all maintenance for the system. Maintenance reports will be provided on an annual basis or by request of the CIieM. 3. Certify and documeM all repairs to the equipment. 4. Perform other services that are incidental to the services specified here including facilitating emergency repairs in the most expeditious and cost effective manner at an additional cost as requested by Client. 5. Pump maintenance to be performed in accordance with manufacturer's specifications by subcantredor and invoiced . by them directly to the Client. SINGLE HOME FAST SYSTEM MAINTENANCE 1. Standard maintenance as follows: a. Check generel condition/appearance of Single Home FAST System,clean media as required. b. Testing of the sludge depth in the septic tank yearly. c. Inspection,power testing and lubrication of the air blower in accordance with manufacturer's recommendations. � d. Inspection of the alartn system. e. Notify owner of any problems. f. Compiete and maintain service report file. 2. Maintenance frequency as follows:Semi-annual Operation and Maintenance visits to pertortn standard Single Home FAST system maintenance. 3. Sampling: Collect annual field testing for pH,turbidity,temperature,and dissolved oxygen. �� 4. Reporting:Prepare and submit summary reports to tF�e DEP,the local Board of Health,and Bamstable County Department of HeaMh and Environment. NOTES: 1. Coastal Engineering will perform no proeedures requiring confined space entry. 2. Services under this contrect specificalty do not include or cover any responsibility for sysbem malTunction attributed to process design,equipment specified and/or installations as provided by otliers. 3. CIieM must provide access to all System components at time of the O&M visit 4. This service wntract assumes year round ocwpancy of the dwelling or facility.The Owner sFqll notify CEC if xcupancy becomes seasonal. 5. CEC will notify the appropriate authority of any event of elecVical or mechanical failure within the treatrnent system,or of any event which may adversely affect the perfortnance of the treatrnent aystem. 6. In the event that the system alarm is acGwated and the system faile,the OWNER shall notlfy CEC who shall notify tt�e DEP and Board of HealtF�within 24 hours. Corrective action shall be taken immediateiy.
