I am ia receipt of your coaespondence dated January 29,2015. The Department has requested that I"submit a detailed .
report that summarizes the necessary procedises you will perform m the fut�se to locate and identify all components and
feat�ses of a system m complimmce with the 310 CMI215.000":
I wIll continue to follow the process of obtaming preliminary information gathering at the local health&building
deparhnents,conservation commissions,water departmems and waste water h�eatment facilities. As I have done in the past
an mterview with the properiy owner will be conducted in conjimction with the preliminary informarian gathering. If the
subject property does not have public records available and in the event that the properry owner(or occupant)cannot provide
mmy direct tangible information then the following procedures will be applied.
Procediae I•Dwellinec/�onerties that do not have town depamnent records and the water service is off or unavailable I will
require the property ownar to consent to a third party licensed plumbing contractor m open all waste lines exiting the i
dwelling. I wID use the combination of a septic inspecrion pnsh=line camora and undergroimd locating equipment !
(transmitter&receiver)to hace out of the building's sewer or ezit line to the initial septic system component The inspection
camera will be used first to visually identify the primary system component The traosmitter and receiver will then be used to
conduct a field sweep or search�d find. '17�is method requires a systematic field pattern,with the electronic locator,to find I
where the transmitter is emanating from below the groimd(our company c�urenUy owns and use these tools). The septic
component will then be opened�d inspected
Procedure II•Dwellines/oronerties That have water servi6e available I wiU use"flushable"tansmitters wlrich are sent out of
a dwellings batluoom(sj to locate the piimary component. I will then follow the field sweep or search and 5nd process. If
the dwelling contains a grey wazer system then I will requite a th'sd party licensed contractor to open the idenrified weste
line(s)and follow Procediue I: In both caees water testing will be done to observe that there is unrestricted flow between the
dwelling and primary septic system component.
Procedure III:Com�onent location(sl after the primarv. I will continue to use the same combination I have in the past of
using both the inspection camera and transmitter/receiver to field locate�d open the component(s)beyond the primary.
Field locatine difficult sites:in some cases cesspool systems use Orangeburg piping or light duty PVC piping as sweeps in .
place of exit tees. In these eircumstances it may not be possible to place a locating device into the sweep openmg and out
thmugh the exit line. In tliese case it is.my axperience that field probing or wates probmg to the sides of the exit line is
required in a systematic pattern until the next component is field located. The objective is to locate the next component and
not compromise the integnty of the older piping network. In some cases a machina is used in conjunction with hand
excavarion to"trace"a lme to the next.sepric system component. The objective is to reach the disin'bution box or next.
leaching area without damaging the system or distutbing the surroimd soil treahnent area(s).

j 2013. (Oceanside Septic Seryices(OSS)hired ow compmmy to conduct a septic rospection and report.:OSS�vas.to.have '
' located aad opened all of the septic system components for our company to.inspect. WLen I azrived at the site�a smgle � .� �. I
i cesspool had been locatad and opened by OSS. As noted on the 5/3/13 septic inspection zeport(pages 9& 13}the dwslling . .: �
contained two exit lines. The initial cesspool;that had been opened for the inspector,contained two inlet lines(1-4"'RVC an� . i
� 1-4"Orangeburg Pipe(coal taz impregnated wood fiber pipe). The inirial cesspool contained a 6"layer of dried solid

the dwelling and the primary-component Water was not present atthe time of the field inspection and therefore a.flushmg �. , - .
test could not have been conducted.
The first component had an exit Ime so I wnducted a seazch and find(which OSS was to have done)and located two :
additional components. The two additional components were opened and inspected. On page 14 of the repor[it is noted•that:
the last Overflow CP3 did not show signs that liquid had ever reached that component I did not notice�y observed signs of
backup,breakout or liquid ponding at the site or within the dwellings besement at the time of the inspection.
When I wes contacted by the ciurent owner(Lauren Grosner)and told tUat there was a problem.with the septic system I
offered to help. I provided her with the name and pLone number of Oceanside Septic Services mmd exp]ained Oceanside ,
Septic Services had hired us to conduct the septic inspection services. OSS was a licensed septic hauler and could conduct
Health Deparlment permitted repa4 work. I was available to meet on site with OSS if essistance was requestad. I was not
contacted about the project after t3iet conversation.
V✓e were contacted in the.spring of 2014 by the Grosner's Attomey,Michael Pierce,and told that the Grosner's wanted
financial compensation for tlie work done to theu septic system and on their property. Ois office requested and received the
work orders and contracts the Crrosners signed for the work that was doue on their septic system. Ois review of the work
� conducted and the scope of service delivered to the Grosners showed that a line repair and additional work had been . �
conducted by Oceanside Septic Services and Comerstone OuWoor Living,LLC(a l�dscape constuctian business). The
scope of the services provided was beyond the repair required Based on the informarion provided it allowed far us to.make a
financial offer to thea attomey. Ois�ancial offer was presented to their atzorney. On hme 9 2014 Attorney Pierce
contacted me d'uectly and said out financial offer was not accepted�d there was no co�mter offer by the Grosners. Attomey
Pierce said he was not instucted to take any fiuther action.
I take the position of being a septic system inspector seriously and try to conduct my work m a moral and ethical way at each
and every job I am on. I have made serious investments in my professianal development,education and equipment to be a
responsible Title 5 System Inspector. It has never been my stsndard to take the easy way through any inspecCionnor would I
intentionally not do a job to the best of my ability. The summary presented detaiLs the necessary procedures I will perform to
idenrify all components and features of a system in compliance with 310 CMR 15.000.
