Please find enclosed the Boazd of Health Approval/Tluee Bedroom Deed Restriction for the
above address which is required as part of the proposed renovations approved by the Yazmouth
Boazd of Health on June 16,2014. The Deed Restriction must be recorded and stamped with the
new Registry recording information at the Barnstable County Registry of Deeds,Route 6A,
Barnstable. A copy of the stamped Restriction must then be retumed to the Health Department
as proof of the recording prior to issuance of the building occupancy permit for the approved
renovations.
