MEMORANDUM OF UNDERSTANDING
The Nutrition Program for the Elderly of Elder Services of Cape Cod and the Islands,
Inc., whose offices are located at 68 Route 134, South Dennis,Massachusetts and the
Yarmouth Council on Aging do hereby agree the ldtchen and dining facilities of the
Yarmouth Conncil on Aging located at 528 Forest Street be available each Monday
through Friday for the preparation of serving of congregate meals and meals-on-wheels to
elder citizens as outlined by Title IIIC of the Older American's Act.
Tlris MEMORANDiJM OF iJNDERSTANDING shall be effective October 1, 2003,by
and between Yarmouth Council on Aging and Elder Services of Cape Cod and the
Islands, Inc.
This Agreement sha11 be in force until September 30,2004 unless amended by either
party or either party issues a thirty- day(30)notice of cancellation.
The kitchen azea and restrooms shall be available for the Nuhition Program each
Monday through Friday from 8:00 a.m. to 2:00 p.m. The dining room shall be
available each Monday, Tuesday,Wednesday and Friday&om 10:45 a.m. to 1 p.m.
each day.
The Nuhition Program shall have use of all ldtchen equipment. The Nuhition Program
shall supply serving equipment, utensils, cleaning supplies and liners for waste
receptacles for kitchen and dining area.
'The Nutririon Program shall be responsible for cleaning the stoves,refrigerator, freezer,
and proper disposal of trash after use by the Prograzn. The Yarmonth Council on Aging
is responsible for cleanup when the facilities aze in use for other activiries.
The Yarmouth Council on Aging provides locked storage space in the kitchen and next
to kitchen areas and is responsible for trash removal and snow removal for ease of access
to the building by the senior citizens. The Council is responsible for custodial care of the
restrooms.
The Yarmouth Council on Aging shall provide a key to the facility to allow access for
the Nutrition Program for the delivery of food.
The Yarmouth Council on Aging and the Nutrition Program agree to meet all
requirements of inspection by fire,building and health officers. ;

The Nutrition Program shall notify the Yarmouth Council on Aging if the kitchen
and/or dining area shall not be needed on a given day. The YarmoutL Council on
Aging shall give adequate notification to the Nutrition Program if the space shall be
unavailable.
The Nutrition Program agrees to provide annually, a current Certificate of Insurance
evidencing Professional and General Liability and Worker's Compensation coverage.
