Subject: April I5, 2008 Liquor Hearing—Bass River Golf Course
This memo is to inform you that applicarn Wiliiam Moran must still provide the following to the
Health Office prior to opening:
1. License application for food service and tobacco sales;
2. Workers Compensation information (signed affidavit or Certificate of Insurance
provided by insurance agency);
3. Copy of food ServSafe certification;
4. Copy of Heimlich/Anti-choking certifications.
On February 2Q 2008, the Health Division conducted a routine inspection during the ongoing
renovations at the establishment. The inspection report noted the need for food safety and
Heimlich certificates. The report was signed by Kevin Moran (copy enclosed).
On the liquor license transfer sheet dated February 14, 2008, the Health Division comments
dated February 2q 2008 also iacluded the need for a food service license, proof of workers
compensation insurance, and ServSafe and Heimlich certificates(copy enclosed).
Applicant Bill Moran was informed again this morning of these requirements by Health
Inspector Phil Renaud. Bill Moran was informed that copies of upcoming ServSafe training and
the contact name for Heimlich/Chokesaving courses would be left in the "pick-up area" of the
Health Office for his convenience.
Bill Moran was informed that he would need a tobacco license if he wished to provide the sale
of cigarettes or cigars. A smoking azea will need to be provided outside the establishment. This
area cannot be on the outside patio where there will be food service.

Subject: April 15, 2008 Liquor Hearing—Bass River Golf Course
This memo is to inform you that applicant William Moran must still provide the following to the
Health Office prior to opening:
1. License application for food service and tobacco sales;
2. Workers Compensation information (signed affidavit or Certificate of Insurance
provided by insurance agency);
3. Copy of food ServSafe certification;
4. Copy of Heimlich/Anti-choking certifications.
On February 2Q 2008, the Health Division conducted a routine inspection during the ongoing
renovations at the establishment. The inspection report noted the need for food safety and
Heimlich certificates. The report was signed by Kevin Moran(copy enclosed).
On the liquor license transfer sheet dated February 14, 2008, the Health Division comments
dated Februa.ry 20, 2008 also included the need for a food service license, proof of workers
compensation insurance, and ServSafe and Heimlich certificates (copy enclosed).
Applicant Bill Moran was informed again this morning of these requirements by Health
Inspector Phil Renaud. Bill Moran was informed that copies of upcoming ServSafe training and

Health Office for his convenience.
Bill Moran was informed that he would need a tobacco license if he wished to provide the sale
of cigazettes or cigars. A smoking area will need to be provided outside the establishment. This
area cannot be on the outside patio where there will be food service.
