Deaz Mr. Khurram,
Your business was involved in the sale of tobacco products to a minor on November 20, 2002 during a routine
Retail Compliance Check performed by the Cape Cod Regional Tobacco Control Program and the Yarmouth
Health Department. Previous violations occuned on June 7, 2001 and February 22, 2001. Your business was
notified to have a representative attend a Yarmouth Board of Health Hearing that was conducted on December
16, 2002. No representative showed up at the meeting.
I called the store at approximately 4:30 p.m. and the clerk claimed that Izzat Bais was "no longer here" at the
store. The clerk further claimed that"Aleem" owned the store. I spoke with you today and you claimed that you
have owned the store since March of this year.
1'he Boazd of Health was dissatisfied with the lack of representation for this Hearing and they felt that the
ownership of the store has not been fully disclosed to this department. They decided that because of these
issues, there should be
Suspension of the Tobacco Permit for 30(thirty) days beginning Aecember 17,2002 and no
renewal for 2003 Tobacco Permit uatil a representative has appeared before t6e nezt scheduled Board of
Healt6 Meeting on January 6,2003. You or your representative will be eapected to appear on this date at
4:15 p.m.
Furt6er,after you and your Manager were informed,verbally and in svriUng by t6e Health
Director aud myself, it was observed at 4:00 p.m. today that cigarettes were still on display in t6e store
and I witnessed t6e sale of ciganttes.
