Re: Sewage Disposal System Upgrade

Deaz Mr. Murphy:
Oceanside Septic, Inc. completed the final inspection for the newly upgraded sewage disposal
system at 9 Frost Ave., Yarmouth, MA. The upgraded system consisted of the existing 1000
gallon septic tank, an H-10 Rated DB-6 Distribution Box (not greater than 3' b.g.) and Twenty
(20) ADS ARC36HC Leach C}�ambers in bed configuration. T'he system was installed with the
top of the units on the lugh side 5.5' b.g. so a vent was installed with a chazcoai filter
The existing leach pit was abandoned at the rime of installation of the new system. The leach pit
was pumped dry, filled with clean sand, and compacted.
, The subsurface sewage disposal system installed at the above referenced properiy has been
constructed in substantial compliance with 310 CMR 15.000, all local requirements of the
Yarmouth Boazd of Health, and the approved design plans dated 06/OS/15 as filed with your
office.
