Dear Mr. Forziati:
The Yazmouth Health Department is in receipt of your request to reduce the monitoring
requirements allowed by the MA TiUe 5 Regulations,Innovative/Alternative Technology
Approval Letter for your septic treatment system at the above address. Upon a review of your
file,your records indicate that your system,installed on November 30,2004,has been under
contract and monitored on a regulaz basis per the inspection reports,as required by the
Department of Environmental Pmtection(DEP)and the Town of Yarmouth Approval Letter
dated October 15,2003.
This Depardnent hereby approves your request fo reduce the maintenance and monitoring

allowed in the DEP Policy/SOP/Guideline General Use Treahnent #BRP/DWM/PeP-P06-1
dated January 1, 2006. Testing requirements wiR consist of pFI,DO(Dissolved Oxygen),and
Turbidity for the life of the system, or,until such time that there is a revision in the DEP Policy
governing the SeptiTech system.
