Dear Mr. Chow,
On October 16, 199'7, I performed a routine food service inspection at your place of business.
During the inspection, several violations were noted dealing with food handling/storage issues and
general cleanliness and maintenance of the facility. I retumed for another inspection on November
6, 1997 and found the restaurarn basically in the same condition. I reviewed my Food Establishment
Inspection Report with you, and discussed a timetable for repairs to be completed.
On January 8, 1998, I performed a third inspection of your restaurant and found many repeat
violations. The serious violations that were noted aze:
-Kitchen ceiling had a large hole in it with water dripping from it.
-Kitchen walls needed to be cleaned and small holes repazred.
- Insulation was exposed around the walk-in units in the krtchen.
- Food was uncovered or cloth napkins were being utilized as food covers which is prohibited.
- Bags of food not stored 6 inches off the ground.
- Employee restroom in poor condition.
During your administrative hearing held with Health Director Bruce Murphy on July 24, 1997, you
were told to implement a cleaning schedule and keep the restaurant m a sacutary condirion. A
cleaning schedule has not been msUtuted and repairs have not been completed.
You aze ordered to attend the Board of Health meeting on Monday, February 2, 1998, at 3:30 p.m.
in the Town Ha11 Hearing Room to discuss your repeat violations. At this meeting the Board of
Health will deternune if your food service pernut should be suspended, modified or revoked.
Please provide written documentation conceming cleaning and maintenance schedules and food
handling/storage procedures.

Dear Mr. Chow,
On June 24, 1997, Health Inspectors Peter Bryanton and John Sweklow perfocmed a routine food service
inspection at your place of business. During the mspection,several violations were noted and e�lained to the
manager on site at the Ume,Mr.Diacon Chow. Senous violations are listed as follows:
1. Meats and poultry stored unrefrigerated;
16. Equipment and utens�s not cleaned or saoitized;
32. Toilet facilities in poor condidon;
7. Food stored on floor of walk-in;
1. Cut vegetables stored unre&igerated;
19. Food contact surfaces not clean or samUzed.
It was aLso explained that we would ietum m two(2)weeks to reinspect the restaurant. I asked Mr. Chow if this
wa�ld be�a►gh time to make necessary changes. He stated that tkte problems would be taken care of. When
we reWmed two(2)weeks later,the Idtchen was bas�cally in the same condihon as when we had left. We noted
all the violatious and explamed to Dixoa that a hearing would be in mder to discuss food handling problems with
the Director of Health.
Mr.Chow,ova'the past two�d�e half(2Y2)yeais as Health Inspector for the Town of Yarmouth,I have Iried
to work with you and your stafF on proper food handling and cleaning techniques. In fact,on the June 24th
inspection, I suggested thai Dixon ereate a"holding table"for food items cunng,mstead of stonng them on
boxes throughout the kitchen. I also lained ihat organization and appearance is cntica( to inspectors.
Apparentiv my suggesti�s wae not appli�ihaefine,I beheve it�s necessary for you to explain to the Director
of Healfh�nxce�Mu�phy,your mteahons to instiiute a food handling and sanitauon program that will resolve
your curcent problems.
An administrative hearing has been scheduled for you on July 24,199'7 at 11:45 a.m. Please provide written
documentation conceming cleaning and food handlinglstorage procedures. Failure to ap ar or provide the
requ�information will result in a Board of Health heanng where the future of your f�service permit will
be discussed.

NATIONAL RBSTAIIRANT ASSOCIATION COIIRSE IN CSINHSE
Here is the latest addition to the training programs from
ESI University. We are now offering the SexvSafe (NIFI)
Applied Foodsenrice Sanitation: National Restaurant
Association course in Chinese. The course will be taught
by certified instructors who are fluent in Mandarin and
Cantonese. We are one of the only companies in the
state able to conduct this course without the use of an
'. interpreter.
The National Restaurant Association� s ServSafe Applied
Foodservice Sanitation (fornierly NIFI) Course in Chinese
by ESI University is a two day, 16 hour program. It
consists of assignments and a final. examination. The
; participant will receive a certificate from the National
Restaurant Association when they pass the final exam with
; a score of 75g or higher.
The ServSafe Course introduces the Hazard Analysis
Critical Control Point System and other ways of
preventing food-related illnesses . It will also help
develop and implement food safety techniques . This will
enable the protection of employees and customers,
prevent food safety errors, increase profits and maintain
quality. Some of the many topics covered include:
1. Sanitation and health.
2 . Serving sanitary and safe food.
3 . The sanitary and safe food environment .
4 . Managing a sanitary and safe food service .
Aside from the direct Chinese instruction, the course
will provide training material, visual aids, and handouts
in Chinese. We are excited about the newest training
