Dear Mr. Chow,
On June 24, 1997, Health tnspectors Peter Bryanton and John Sweklow performed a routine food service
iaspectio�at your place of business. During the mspection,several violations were noted and e�cplained to the
manager on srte at the time,Mr. Dixon Chow. Senous violations are fisted as follows:
1. Meats and poultry stored unrefrigerated;
16. Equipment and utensils not cleaned or sanitized;
32. Todet facilities in poor condition;
7. Food stored on floor of walk-in;
1. Cut vegetables stored unrefrigerated;
19. Food contact surfaces not clean or sanitized.
It was aLso acplained t�at we wauld ieturn in two(2)weEks W reinspect the restaurant I asked Mr. Chow if this
would be a�wgh time to make necessary changes. He stated that the problems would be taken care of. When
we retwned two(2)weeks lat�,fhe ldtchea was basically in the same condition as when we had left. We noted
all the violations and explamed to Dixon that a hearing would be in order to discuss food handling problems with
the Director of Health.
Mr.Chow,ovex the past two and me half(2Y:)yeazs as Heal�Inspector for the Town of Yazmouth,I have tried
to work with you and your staff on proper food handling and cleaning techniques. In fact,on the June 24th
inspection, I suggested that Dixon create a"holding table"for food items cunng,instead of storing ihem on
boxes throughout the Idtchen. I also lained rhac organizaaon and appearance is criacal co inspectors.

of Health$nx�e�Iviurphy,your inteahons to rostitute a food handling and sanitation program that will resolve
your current problems.

documentation conceming cleaning and food handling/storage procedures. Failure to ap az or provide the
requ�infamati�will result in a Boazd of Health hearing where the future of your f�ervice peanit will
