Deaz Mr. Chow,
On May I, 2002,I reviewed your old inspec[ion reports and i felt that an inspecrion of your
resfaurant was necessary. I performed a complete inspec.�t.ion and found ihe following Health Code
Violadons/Problems:
1. The floor had a general build-up ranging from minor to major and was especially bad under
the dish machine.I recommended a good,thorough cleaning of the floor and baseboards using
hot soap and water.
2. The dish utachine leaked at the detergeiR feed and the rinse produc[was low enough for the
alazm to sound.The rinse tempetature reached 180°F only after five consecuUve runs. I
informed you tk�at the technician was needed to fix the leak and tiun up the booster to
maintain proper temperatures.
3. The hood fans had major build up and needed cleaning immediately.Hood filters were
missing from the main cooldng azea in front I ezcplained the nced to have all filters insfalled
and the requirement to t�ave a clean hood fan.
4. The dry storage area was extremely cluttered and was in need of sfraightening out. I eacplained
tt�at the floor azound the flour bags would auract vermin.
I informed you that all these violations would need to be rectified and tLat I would be back witltin two or
tLree weeks. I re-inspecte,d your restaurant on June 3,2002 and found the following:
1. The floor was in the same condition as the previous inspectioa I noticed cracked and loose
61es that I did not nolice before.
2. The dish machine was still leaking,low on rinse agent as indicated by the alazm,and the rinse
tempeiature never made it past 150°F.
3. The hood fans were in worse condition tkian before; grease had collected in long hanging
s[rands on the Ansel unit piping.'I'his was a Fire Code violatlon.
4. The dry storage area was in the same condition as before.
5. Potentially Hazardous Foods(PHF's)in the middle walk-in refrigerator were not dated.
Chicken and beef were at an acceptable temperatiue but it was not clear how long they had
been refrigerated.
6. An employee(Asian,50's,female)was cooking behind the line and was wiinessed as leaving
her unwrapped food on a cuUing board at the line.Ttds is cross-contamination and a serious
Health Code Violation.
These Health Code Violations and Isaues ueed to be addressed and you are ordered to attend
an Administrative Hearing echeduled tor Thursday June 13,2002 at 11:00 am.in the Health
Deparhnent Director's Office.
Please provide written documemation conceming a repair schedule and general cleaning schedule
