Captain Pazkers Pub, Inc. 668 Route 28 West Yarmouth, MA 02673 Re: 2013 License/Permit Application Deaz Mr. Manning, Thank you for submitting the 2013 application for your establishment's food service and common victualler permits issued through the Health Deparhnent. Please note that a copy of the Food > Protection Manager's certification, as well as copies of Heimlich Maneuver certifications,aze required to be submitted with the application. All food service establishxnents are required to have at least one full-time employee who is certified as a Food Protection Manager, as defined in the State Sanitary Code for Food Service establishments, 105 CMR 590.000. All food service establishxnents with 25 seats or more must have at least one employee trained in the Heimlich Maneuver on the premises at all times. Please note that the Health Department cannot use past yeaz's records,as we aze unable to verify if those staff inembers aze still under your employment. As soon as our office receives the above noted certification copies,we will be able to compiete the processing of the food service and common victualler permits.
